Use your daily journal to document how you feel each day. Document your activities and your limitations. Be as specific as you can. Write anything physical or mental that is challenging you.
Ask your doctor to document your symptoms and disabilities. Help him/her understand that you need evidence of all sort of disabilities such as depression, dizziness, forgetfulness, fatigue, and other symptoms you may suffer. Please ask your doctor to be as specific as possible.
