Keep records of all your symptoms or illness or injury and how it is impacting your life. Try to keep a daily journal on how you feel and how your injury or illness affects your activities.

Use your daily journal to document how you feel each day. Document your activities and your limitations. Be as specific as you can. Write anything physical or mental that is challenging you.

Ask your doctor to document your symptoms and disabilities. Help him/her understand that you need evidence of all sort of disabilities such as depression, dizziness, forgetfulness, fatigue, and other symptoms you may suffer. Please ask your doctor to be as specific as possible.

There are certain medical records that are essential to process your claim. Such records include but not limited to:

Statements from your doctors

Record of your visits and diagnosis from your doctor or hospital or clinic. Generally any documentation about your health is helpful if it provides information about the causes or treatment of your disability.

Keep a record of your doctors, clinics, hospitals you have attended for any treatment relating to you disability.

Keep record of all prescriptions  you take. It is helpful to keep copies of the actual prescriptions.
